4.	Ketentuan Persyaratan Terms and Conditions	a.	Persyaratan Sertifikat Standar meliputi: Standard Certificate requirements include:
			1) pengajuan permohonan secara elektronik; 2) Nomor Izin Berusaha dengan Klasifikasi Baku Lapangan Usaha Indonesia yang sesuai dengan Sertifikat Standar yang dimohonkan; 3) susunan pengurus, daftar pemegang saham dan/atau penyertaan modal, dan daftar pemilik manfaat sampai dengan penerima manfaat akhir; dan 4) dokumen yang memuat daftar tenaga ahli yang bekerja secara purnawaktu dalam bentuk tabel yang dilengkapi dengan bukti pendukung pemenuhan tenaga ahli yang harus sesuai dengan bidang sertifikat standar yang diajukan. 1) electronic submission of the application; 2) a Business Identification Number (NIB) with an Indonesian Standard Industrial Classification (KBLI) code corresponding to the Standard Certificate being applied for; 3) the composition of the management, a list of shareholders and/or capital participation, and a list of beneficial owners up to the ultimate beneficial owner; and 4) a document containing a tabular list of full-time experts, accompanied by supporting evidence of compliance regarding the experts, which must align with the field of the Standard Certificate being applied for.
		b.	Persyaratan Perubahan Sertifikat Standar dalam rangka penambahan bidang meliputi: 1) persyaratan administratif dan teknis sebagaimana dimaksud pada huruf a angka 1) dan angka 2); dan 2) salinan dokumen Sertifikat Standar sebelumnya. The requirements for amending the Standard Certificate to add a field comprise: 1) administrative and technical requirements as referred to in letter a, item 1) and item 2); and 2) a copy of the previous Standard Certificate document.
		Dalam hal dokumen atau data dan/atau informasi telah tersedia dalam sistem perizinan online (OSS), dokumen atau data dan/atau informasi tidak perlu disampaikan Pelaku Usaha. Dalam hal dokumen atau data dan/atau informasi mengalami perubahan, pelaku usaha melakukan perubahan data dan/atau informasi dalam sistem perizinan online (OSS). If documents, data, and/or information are already available in the online licensing system (OSS), the Business Actor is not required to submit them. In the event of any changes to such documents, data, and/or information, the Business Actor shall update the data and/or information within the online licensing system (OSS).
