Ketentuan Verifikasi Verification Requirements |

1) |
Verifikasi dilakukan terhadap kesesuaian dokumen persyaratan; Verification is conducted
regarding the conformity of the required documents; |

|
2) |
Verifikasi permohonan dilakukan melalui tim evaluator perizinan; Verification of the application
is carried out by the licensing evaluation team; |
|||
|
3) |
Verifikasi terhadap kesesuaian dokumen persyaratan dilakukan dalam
jangka waktu paling lama 14 (empat belas) hari kerja sejak permohonan
diterima dan lengkap; Verification of the conformity
of the required documents shall be carried out within a maximum period of 14
(fourteen) working days from the date the application is received and
complete; |

4) |
Apabila berdasarkan verifikasi permohonan dinyatakan sesuai,
permohonan disetujui; dan If based on verification the
application is declared appropriate, the application is approved; and |
|||
|
5) |
Apabila berdasarkan verifikasi permohonan dinyatakan tidak sesuai,
permohonan ditolak disertai alasan penolakan. If based on verification the
application is declared unsuitable, the application is rejected along with
the reasons for rejection. |

Ketentuan Pengembalian Return Policy |

|

|
1) |
pengajuan permohonan secara elektronik disertai alasan pengembalian
izin; dan electronic application
submission accompanied by reasons for returning the permit; and |

2) |
salinan bukti penyampaian laporan triwulan dan tahunan. Copy of proof of submission of
quarterly and annual reports. |

Ketentuan Kewajiban Obligation Provisions |
Kewajiban Pemegang Izin Pengangkutan dan Penjualan: Obligations of Holders of
Transportation and Sales Licenses: |

|
a. |
melaksanakan kegiatan pengangkutan dan penjualan sesuai dengan
ketentuan peraturan perundang-undangan; carry out transportation and
sales activities in accordance with the provisions of laws and regulations; |
|||
|
b. |
menyampaikan laporan kegiatan pengangkutan dan penjualan sesuai
dengan ketentuan peraturan perundang-undangan; submit reports on
transportation and sales activities in accordance with the provisions of laws
and regulations; |
|||
|
c. |
mematuhi ketentuan peraturan perundangundangan di bidang lalu lintas
dan angkutan jalan apabila menggunakan fasilitas umum; comply with the provisions of
laws and regulations regarding road traffic and transportation when using
public facilities; |
|||
|
d. |
melakukan kegiatan pengangkutan dan penjualan komoditas mineral atau
Batubara yang berasal dari IUP, IUPK, IUPK sebagai Kelanjutan Operasi Kontrak/Perjanjian,
IPR, KK, PKP2B atau Izin Pengangkutan dan Penjualan lainnya; conducting the transportation
and sale of mineral or coal commodities originating from a Mining Business
License, Special Mining Business License, Special Mining Business License as
a Continuation of Contract/Agreement Operations, People's Mining License,
Contract of Work, Coal Mining Concession Agreement, or other transportation
and sales licenses; |

e. |
mematuhi segala kewajiban lainnya sesuai dengan ketentuan peraturan perundangundangan;
dan comply with all other
obligations in accordance with the provisions of laws and regulations; and |
|||
|
f. |
tidak melanggar larangan yang telah diatur sesuai dengan ketentuan
peraturan perundangundangan. does not violate prohibitions
established in accordance with the provisions of laws and regulations. |
